Hi please could someone help give me some ideas for question 11

describe appropiate methods of monitoring,reviewing and evaluating care plans

Is it asking me to write about how monthly reviews are done and more formal reviews are done every 6 months or just how by observing clients and identifying when they may need to have their plan updated etc ??????????????
