Sorry still not sure about this question......

Describe appropriate methods of monitoring, reviewing and evaluating care plans.

In my work setting key workers who work more closely with their particular client do monthly reviews which are useful as a general overview of a clients health and well being for a particular month,a more formal review takes place every 6 months although care plans are an on going review process and need to be flexible to change. When working with clients it is important to be vigilant to an individuals changing needs which may affect the relevance of their care plan and result in changes having to be made e.g a client seems to find it harder to weight bear so how they are assisted to move may need to be reviewed.

Thats what I wrote so far but do I have to include about other reviews with other key people like medication reviews etc and not sure about the evaluating bit???????is that just recording outcomes of changes???? sorry to be a pain this site is a godsend as is NVQman but for some reason cant register on there.

Hi flick24,

Again a good answer but you could include who attends the reviews, particularly the formal 6 monthly reviews.

You have included evaluation but you haven't used the word. You could change "care plans are an on going review process" to "care plans undergo regular monitoring and evaluation and any required changes are discussed in the reviews".

You could include medication reviews but you don't need to.
