Hi, can anyone help? Describe 2 ways of making sure that the history, preferences, wishes and needs of an individual using the service are recognised in support or care plans. Could one be to discuss it with the service user? I don't fully understand the question

Yes! And you need to make sure you follow plans, report and record any changes and ensure you keep up high standard of work
