Hi, some ideas, person-centred support, communicating with the individual using their preferred method, consistency of support - all involved are working in the same way to avoid causing confusion or uncertainty for individual, give up-to-date information, be aware of any possible change in behaviour, ensure individual is involved, supported to take control, feel included & empowered in the process, ask the individual how best they can be supported, support planning, who is responsible for what, liaising with family & friends if appropriate (with individual's consent. ) Ask for advice, support, guidance from your manager, other professionals & those experienced in the area you are supporting individual with eg moving home, bereavement, break-up of a relationship, starting college, new job, etc. Think about how you would like to be supported through change. |
