I am no expert but anwsering your question possibly if they leave something electrical on which could cause a fire. Possibly smoking not putting a cigarette out properly. Also if they do not report something that the worker is not aware of because for example if the client noticed the carpet in the home is dangerous they would also have a responsbility to inform someone. I really hopes this helps or at least guides you. Possibly cooking a meal and left the frying pan on with burning oil.

Just thought of something else if they were looking after their own medication and left it hanging around where someone else could pick it up and take it and overdose.
