Use examples from your workplace. Explain when you update records, after shift? the next day? next time you're on shift? Do you record facts or opinions? do you sign and date them? Do you leave them out in lounge or lck them away. When you're updating them do you do it in area where you can be overlooked or somewhere quiet and private? Can you lock place where records are kept, if they are held on pc is it password protected or have some form of security?
Look for the obvious answers and you won't be far wrong.
