Generic Form Set for Adult Care
A set of 32 forms for use within the Adult Care sector. These forms meet the requirements of the Essential Standards of Quality and Safety issued by the CQC which provides guidance for compliance with the Health and Social Care Act 2008(Regulated Activities) Regulations 2010.
There are forms that you will be familiar with and there are some new additions which reflect the new guidance requirements.
W&P have designed the new forms in order to assist managers in demonstrating compliance within the new guidance requirements. The Code of Practice for Health and Adult Social Care on the prevention and Control of Infections and related Guidance, has specific requirements regarding THE INFECTION PREVENTION AND CONTROL LEAD post holder which is a new requirement within this code of practice under criterion 1. This role has various responsibilities which include an annual statement which has to be returned to the Care Quality Commission.
As part of this Generic Form set there is an Infection Prevention and Control Annual Statement which covers all of the requirements necessary in the report. The post holder needs to be a designated, named person within any Adult Social Care setting, but it is a proportionate role determined by the size of the organisation.
Another new form is Consent to Care and Treatment, which demonstrates Outcome 2, Regulation 18 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010.
As part of these new forms there is also a Care Quality Commission Notification Letter. This is a new requirement under the Care Quality Commission (Registration) Regulations 2009. This set of regulations go hand in hand with those contained in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010. The CQC regulations are mainly to do with requirements regarding notifications of incidents. Outcome 20 in particular needs to be adhered to. Outcome 20 in particular needs to be adhered to, as this is the regulation where notification of other incidents is a requirement. This includes deaths and abuse situations.
